Our Services

.We Care and Support is designed to enable individuals with a disability to live independently, offering appropriate support to help them manage their own tenancy and achieve greater freedom and control in their lives. We aim to improve the quality of life of people we support.

Supported living refers to schemes that provide personal care to people as part of the support that they need to live in independently. The personal care is provided under a separate contractual arrangement to those for the person’s housing. The accommodation is often shared, usually as a small group, but can be single household.

Supported living enables adults with support to have their tenancy with the help they need to be independent and benefit from a greater level of autonomy as far as their environment is concerned. It allows them to choose:

 

  • where they want to live
  • who with
  • how they want to be supported
  • what happens in their own home.

Our staff are fully trained and take a person-centred approach to planning and delivering each individual’s support.

This means that, when we work with a person and their family, we listen to what they want and what matters to them, so they are always at the very heart of conversations and decisions about their support, and how it is planned and provided.

Together, we create a unique and tailored ‘personal support plan’ for each individual, that outlines the type of support they receive, how much and how often.

Our Services

We provide supported living services across London and neighbouring boroughs. Our fully trained staff can help with things like:

  • personal care – health and wellbeing, washing, preparing meals, medication
  • running a home – maintaining a tenancy agreement, budgeting, paying bills
  • healthy living – help with making healthy lifestyle choices
  • household tasks – cleaning, laundry, cooking
  • building links with the community – developing friendships, maintaining personal relationships, contact with family and friends
  • leisure activities – socialising, going on holiday
  • education and employment – identifying opportunities, applying for college or jobs, arranging training.
  • collaboration with Multi-Disciplinary Team – to facilitate collaboration between professionals such as Social Workers, Care Coordinators, Consultants etc and hence improve outcomes for people and communities
  • access Services and attend Appointments – help to get to appointments and collaborate with other organisations or access services such as GP Services, health centres etc to ensure good health and well-being for people we support.

We achieve this by offering tailored assistance, focusing on skill development and goal achievement. Our proactive approach involves a collaborative team dedicated to positive outcomes. Our clients, who may have autism, mental health issues or learning disabilities, have often faced restrictions in the past. We prioritise their experiences, aiming to rebuild trust and offer personalised support packages, including behaviour plans and goals.

Our aim is to enhance independence and quality of life by empowering those in our care. Through person-centred planning and the PBS Framework, we engage with individuals, valuing their preferences and introducing new experiences. Our thorough assessments help create personalised skill-building plans, promoting social independence gradually.

We Care and Support are dedicated to ensuring the growth of those we assist, whether it’s mastering new skills in cooking, road safety, or engaging in community activities. Using evidence-based assessments, we identify areas needing attention and monitor progress through data-led decision-making. Our support covers various domains, from health and safety to socialisation, communication, and daily living skills. Areas of focus include communication, self-care, support with medications, meal preparation, social skills, budgeting, and exploring new hobbies.
