Transforming Care

The Transforming Care programme revolves around enhancing health and care services to enable more individuals to reside within their communities, receiving appropriate support near their homes. This approach aims to reduce the necessity for hospital visits for their care.

The initiative for this transformation, known as ‘Building the right support’, is part of a UK-wide plan enabling this shift. Across England, there are 48 transforming care partnerships (TCPs) established to facilitate and bolster this endeavour.

WCAS are proud to support the Transforming Care programme in its mission to help individuals thrive in their homes rather than in hospitals.

Autism care and support

People with autism have unique strengths and abilities connected to their diagnosis. Sometimes, these qualities might not be easy to see right away. Our team of experts focus on seeing beyond the challenges and highlighting the positive sides of autism.

We’re here to help with various aspects like social communication, handling social anxieties, managing emotions and understanding the world better.

Since everyone is different, we offer lots of different kinds of support, which is what makes our approach special. Here are some ways we can help individuals with autism:

  1. Special places to live for adults with autism, starting from the transition age (16-25) to later years.
  2. Specialists in Positive Behaviour Support (PBS) who can help with challenging behaviours.
  3. Advice on funding, housing options, and more from our referrals hub, open seven days a week.
  4. Thorough assessments and plans for support, involving many teams and experts.
  5. Access to special activities and spaces at home or in the community.
  6. Innovative ways to communicate, both verbally and non-verbally.
  7. Regular updates for families and professionals through monthly newsletters.
  8. Help with emergency housing and short breaks.
  9. Guidance on living independently with an autism diagnosis.

At WCAS, we have the expertise to assist young individuals and their families in navigating the shift from children’s to adults’ services. This change occurs when a young person with a learning disability, eligible for social care support, turns 18 and starts receiving assistance from adult-focused social care services.
